Quick Answer

What is a PRP injection for the knee?

PRP (platelet-rich plasma) is a concentrate of platelets prepared from your own blood and injected into the knee. Platelets release growth factors that may reduce inflammation and pain. It is a day-care procedure — about an hour, no admission, and you walk home the same day. At ONE Healthcare Clinic, Tirumalagiri, Secunderabad, PRP is offered for early-to-moderate (Grade 2–3) knee arthritis and some tendon problems, after an examination and X-ray by Dr. Sumit Dubewar.

Who may benefit

  • Knee osteoarthritis up to Grade 2–3 with pain despite exercise and weight management
  • People who want to delay knee replacement and still have reasonable joint space
  • Patellar tendinopathy (jumper’s knee) not settling with rehab
  • Selected cartilage and meniscus problems, as part of a rehab plan

When PRP is not advised

  • Grade 4 (bone-on-bone) arthritis — PRP is unlikely to help; other options are discussed
  • Active infection in or around the knee, or fever
  • Low platelet count or bleeding disorders
  • Active cancer, pregnancy, or certain blood-thinner treatment (discussed individually)
  • A mechanically locked knee that needs surgery
The Procedure

What happens on the day

1 · Blood draw
A small amount of blood is taken from your arm, like a routine blood test.
2 · Preparation
The blood is spun in a centrifuge to concentrate the platelets.
3 · Injection
Under strict sterile conditions, the PRP is injected into the knee joint.
4 · Home
Short rest, then you walk home the same day. Total time about an hour.

Usually 1–3 sessions are given, about 4–6 weeks apart, depending on severity. Some patients receive PRP combined with hyaluronic acid. For selected Grade 2–3 knees, bone-marrow (BMAC) or fat-derived (mFAT) concentrate prepared from your own tissue may be discussed.

Before & After

Preparation and recovery

  • Stop anti-inflammatory painkillers (e.g., ibuprofen, diclofenac, aceclofenac) for about a week before and after — they can blunt the platelet effect. Paracetamol is fine.
  • Eat normally and drink plenty of water on the day.
  • Tell us about blood thinners, diabetes control and any recent fever.
  • Full checklist: How to prepare for PRP
Day 1–2
Soreness or stiffness for 1–3 days is common. Ice packs and paracetamol.
Day 3–5
Most people resume routine daily activities.
Week 2+
Restart your strengthening exercises — PRP works best alongside them.
Month 3
Benefit is usually assessed at this stage.
The Evidence

Does PRP really work for knee arthritis?

The evidence is encouraging but not uniform. Several systematic reviews and meta-analyses have found that PRP gives better pain and function scores at 6–12 months than hyaluronic acid or steroid injections in early-to-moderate knee osteoarthritis. However, at least one well-conducted randomised trial (published in JAMA, 2021) found no clear benefit over a saline injection at 12 months. Results also depend on how the PRP is prepared and on the stage of arthritis.

What this means for you: PRP is a reasonable option to reduce pain and improve function in Grade 2–3 knees when exercise alone isn’t enough. It does not regrow cartilage or reverse the X-ray grade, and not everyone responds. Dr. Sumit will tell you if your knee is unlikely to benefit.

Common Questions

Frequently Asked Questions

Is PRP injection for the knee painful?

The injection itself is brief. Mild to moderate soreness or stiffness for 1–3 days afterwards is common and is managed with ice packs and paracetamol.

How many PRP injections are needed for the knee?

Usually 1–3 sessions, spaced about 4–6 weeks apart, depending on the severity of arthritis and your response.

How long does PRP relief last?

In studies of early-to-moderate knee arthritis, benefit has commonly been reported for 6–12 months, sometimes longer. Response varies between individuals and some people do not respond.

Can PRP avoid or delay knee replacement?

In Grade 2–3 arthritis, PRP combined with exercise may reduce pain enough to delay surgery. It does not regrow cartilage, and it is not recommended in Grade 4 (bone-on-bone) arthritis, where knee replacement is usually the better option.

Is PRP safe?

PRP is made from your own blood, so allergy or rejection is unlikely. Temporary pain and swelling are common; infection is rare but possible, which is why the injection is done under strict sterile conditions.
