India gets abundant sunshine all year round, yet vitamin D deficiency is very common here. Low levels of vitamin D can contribute to generalized body aches, muscle weakness and fatigue, and can worsen bone health over time — though it is not a proven direct cause of joint diseases like arthritis. Here's what you should know.

Why is vitamin D deficiency so common?

Even with plenty of sunshine, most people fall short

Vitamin D is often called the "sunshine vitamin" because the skin produces it when exposed to sunlight. Given how much sunshine most of India receives, deficiency might seem unlikely — yet blood tests routinely show low vitamin D levels across all age groups, in cities and villages alike. This happens because actual sun exposure, skin biology, and diet all play a role — not just how sunny the climate is.

  • Limited direct sun exposure — most people spend the day indoors at work, school or home, often behind glass or under artificial light, so the skin never actually gets the sunlight needed to make vitamin D.
  • Sunscreen and fully-covering clothing — sun protection and clothing that covers most of the skin (for cultural, religious or practical reasons) block the UVB rays needed for vitamin D production.
  • Air pollution — haze and pollution in many Indian cities scatter and absorb UVB rays before they reach the skin, reducing effective sun exposure even outdoors.
  • Darker skin pigmentation — melanin, which gives skin its darker tone, also reduces the skin's efficiency at producing vitamin D from sunlight, so people with darker skin often need longer sun exposure for the same effect.
  • Low dietary intake — very few naturally occurring foods are rich in vitamin D (mainly fatty fish and egg yolks), and fortified foods are not yet widely consumed in India, so diet alone rarely provides enough.
  • Other contributing factors — obesity, certain malabsorption conditions, and chronic kidney or liver disease can all reduce the body's ability to activate and use vitamin D.

Symptoms

Often vague — which is why it's easy to miss

  • Generalized aches in the bones and muscles — often in the lower back, hips, thighs and legs
  • Muscle weakness, particularly in the muscles closer to the trunk — noticed as difficulty climbing stairs, getting up from a low chair, or a change in the way you walk
  • Persistent fatigue and low energy that isn't explained by sleep or workload
  • Bones that feel tender when pressed, especially over the shins, ribs or pelvis
  • In children with severe, prolonged deficiency — rickets, with bowed legs, delayed growth, and soft or deformed bones
  • In adults with severe, prolonged deficiency — osteomalacia, where the bones themselves become soft and more prone to pain and fracture

See a doctor promptly if body aches are accompanied by severe bone pain that limits walking or weight-bearing, a suspected fracture after minor trauma, or signs of rickets in a child (bowed legs, delayed walking, bone deformity). These need proper evaluation and should not be managed with over-the-counter supplements alone.

Diagnosis

A simple blood test, plus ruling out other causes

Confirming the Deficiency

  • 25-hydroxyvitamin D blood test — a simple blood test that directly measures the body's vitamin D level and is the standard way to confirm deficiency
  • A detailed history and physical exam — including how much time is typically spent outdoors, diet, and an assessment of muscle strength and bone tenderness
  • Additional blood tests when needed — calcium, phosphate, alkaline phosphatase and parathyroid hormone (PTH) levels help assess how the deficiency is affecting bone metabolism
  • X-rays — usually not needed for routine deficiency, but used when osteomalacia, rickets or a stress fracture is suspected

Ruling Out Other Causes

Because generalized aches can come from many sources, your doctor will also look for other explanations — such as osteoarthritis, inflammatory arthritis, thyroid problems or muscle disorders — rather than assuming vitamin D deficiency is responsible for a specific joint complaint.

Sunlight, diet, and doctor-guided supplementation

Correcting the Deficiency

  • Sensible sun exposure — around 15-20 minutes of midday sun on the arms and legs, a few times a week, without sunscreen, is usually enough for the skin to make vitamin D naturally
  • Vitamin D-rich foods — fatty fish such as salmon, mackerel and sardines, egg yolks, mushrooms exposed to sunlight, and fortified milk or cereals when available
  • Supervised supplementation — vitamin D3 supplements, in a dose and schedule decided by your doctor based on your actual blood level; supplements should not be started at high doses on your own, since over-supplementation can itself cause problems such as high calcium levels

Supporting Bone and Overall Health

  • Adequate dietary calcium, since vitamin D works together with calcium for bone strength
  • Weight-bearing activity such as walking, which helps maintain bone density
  • Treating any underlying condition — such as obesity, malabsorption, or kidney or liver disease — that may be affecting vitamin D levels
  • Periodic repeat blood testing to confirm levels have normalized and to guide any adjustment in supplementation

Correcting a vitamin D deficiency can meaningfully improve generalized bone and muscle aches, fatigue, and bone health — and is important for preventing or managing osteoporosis and osteomalacia. However, it is not an established primary cause of joint diseases such as osteoarthritis or rheumatoid arthritis, so if you have pain localized to one specific joint, it still needs its own proper evaluation rather than being attributed to vitamin D alone.

Vitamin D deficiency itself isn't treated with a specific exercise program, but weight-bearing activity — such as walking, stair climbing or light resistance training — works alongside adequate vitamin D and calcium to keep bones strong. If you have a specific joint or back problem, look for exercises suited to that condition.

A useful clue, not a catch-all answer

In my clinic, I often see patients who've been struggling with vague, generalized body aches for months, going from one specialist to another without a clear answer. A vitamin D check is one of the simplest, most useful things we can do early on — it's inexpensive, low-risk, and genuinely common to find low. But I'm equally careful not to let it become a default explanation. If a patient has pain focused on one joint, or signs pointing to arthritis or another specific condition, correcting vitamin D alone won't fix that — it needs its own proper diagnosis. My approach is to check what's easy and useful to check, treat what's actually low, and keep looking for the real cause of localized pain rather than settling for a convenient answer.
